Available through Magnus Health Portal. Sign in today to get this requirement checked off and/or make those doctors appointments. DUE JULY 20

2. Complete all the required paperwork under Student Health Tracker. Please note that you need 2 non-parent Emergency Contacts for your paperwork to be accepted.
3. STM Nurses will review your information and let you know if anything else is needed.

NOTE: Students may not attend school at STM until all requirements are met and checked by STM Nurses, so get this finished early to allow them ample time to process your documents and responses!
